Middle School Ultimate
All players must have an active membership with USA Ultimate which includes insurance for the player. There are 2 choices:
$20/Yr. = Affiliate Recreational Player (via DEVYL) - for those on a budget.
$45/Yr. = Full Youth Membership - for those who can support USA Ultimate.

During the registration process you will need to upload a signed copy of the USA Ultimate Medical Authorization form.
The form can be downloaded HERE.
The “Name of Chaperone” should be “Montclair Ultimate”.
Registration is done through TeamSnap. If you don't already have a TeamSnap account the sign-up process will prompt you to create one. Parents should create an account first and then add their family members.
We don't want cost to be a barrier to playing Ultimate. Financial aid is available for anyone who needs it. Learn more...
